Slight correction, 10% penalty, not tax. Money in Roth IRA is post-tax, you will not pay taxes when you with-draw

not sure on yearly reporting part. Confirmed money will not be added to your AGI (Annual gross income) per current rules. So, worst case you report that you got the money, but it'll not add to your taxable income
