I have applied using my aadhar card number,,,so don't need to send any documents. antha online data fill cheyyatame,,,aadhar appudu use chesina info and photo neee adhe automatic ga pull chesindhi.

Yes where OTP goes is important.
After sending documents within a week you can get the PAN.
If you give India address some one with a valid photo identity has to be there else they won't deliver.
If US address then it's all fine.

you have to contact the PAN card issuing authority, you can email them which is there on PAN card website. They might ask to send proof of DOB..
